3) Certified copy of degree or equivalent qualification

4) Certified copy of Award Certificate in Pharmaceutical Sciences

5) Certified copy of Academic Transcript

6) Certified copy of evaluation certificate from TCU forstudent acquired degree outside Tanzania

7) A certified copy of Birth Certificate or Passport

8) Current Curriculum vitae/Resume

9) Four recent passport size photographs 2x2.5cm, endorsed at the back by public notary and bears name of applicant at back.
